To thank you for your continuing participation in CHILD, we are hosting our first-ever Winter Giveaway Contest!
- Eight prizes will be given away at each study site (five prizes for CHILD youth, three prizes for CHILD parents), and
- One grand prize will be awarded at the national level.
What are the prizes?
National draw:
- 10th gen iPad ($600 value)
At each study site:
For youth
- 1st draw: Gotrax Vibe Foldable Electric Kick Scooter + Thousand Skate Helmet
($540 value) - 2nd draw: Beats noise-cancelling wireless headphones ($440 value)
- 3rd-5th draw: Gift cards for the Apple Store, Best Buy, Door Dash, Skip, or another vendor of your choice ($200 value)
For parents
- 1st-3rd draw: prepaid VISA card ($300 value)
How do you participate in the contest?
Every CHILD family can gain entries (raffle tickets) for these draws in the following ways:
- Book a CHILD clinic visit: 3 entries
- Complete a CHILD clinic visit (in-person or virtual): 1 entry
- Complete 4 or more child/youth questionnaires: 1 entry
- Complete 6 or more parent questionnaires: 1 entry (this is for the parent prizes!)
- Submit biological samples: 1 entry
Everyone in the Study can participate!
If you have already taken any of these actions, you will have already earned entries.
If the CHILD youth in your family is too young (under 11 years and 9 months of age) to complete their clinic visit or to submit their biological samples, they can still earn entries by booking their visit and completing questionnaires.
For families with youth of age to complete visits and submit samples (11y 9m and up):
- all actions apply
- potential to gain 6 entries for youth + 1 entry for parent
- total potential entries: 7
For families with youth NOT of age to complete visits and submit samples (under 11y 9m):
- visit booking and questionnaire completion apply
- potential to gain 4 entries for youth + 1 entry for parent
- total potential entries: 5
For families who already completed their 13Y visits:
- all actions apply
- potential to gain 6 entries for youth + 1 entry for parent
- total potential entries: 7
Book a visit: 3 entries
Complete a visit: 1 entry
Complete questionnaires: up to 2 entries
Submit biological samples: 1 entry
